The mechanics
Three kinds of glass, three different jobs
The simplest case is a single sheet of annealed glass bedded in linseed putty or held by wooden beads. Old glass and putty out, new glass bedded in, and the sash is good for decades. Much of Iowa's older housing is glazed exactly this way.
Newer windows hold an insulated unit: two panes bonded around a spacer with a sealed cavity between them. That unit is a manufactured item in its own right and is replaceable inside your existing sash. When a window 'fogs', the perimeter seal has failed and moisture has entered the cavity. The haze will not clear, the insulating value is gone, and nothing about the frame or hardware is wrong.
Then there is tempered glass, required by code beside doors, in bathrooms, at low sill heights and in large panes. It cannot be cut after manufacture, so it is always made to measure. That is why glass work here is a measure-first, fit-later job rather than something carried on a van.

Cracked glass and failed putty both escalate, and they escalate into different problems.
Right now
Glass only
A pane or a sealed unit replaced in a sound sash. One component, nothing else touched. The cheapest this will ever be.
Next season
Glass plus water
A crack reaching the edge lets water into the glazing rebate. Putty that is already crumbling does the same. Now the sash rail underneath is wet every time it rains.
Two winters on
Joinery
Water in a rail through repeated freeze-thaw opens joints and softens timber. The glass still needs doing, and now the sash needs repair before it can hold glass. Two jobs instead of one.
Eventually
The sash itself
A rail that has gone soft cannot be reglazed. At that point you are rebuilding or replacing a sash that was entirely sound when the putty first cracked.
A cracked pane in the middle of a window is often genuinely stable and we will tell you so. It is edge cracks and failed putty that are worth moving on, because both let water somewhere it should not go.

Hail, most years
Iowa sits squarely in hail country, and a single storm can pit, chip or crack glass across a whole side of a house. Panes that survive visibly intact can still have lost their seal, which shows up as fogging weeks or months later — which is why we check the elevation rather than the window.

Severe wind events
Iowa is exposed to straight-line wind events of real severity, the August 2020 derecho across Cedar Rapids and central Iowa being the extreme example. Wind-driven debris takes glass in quantity, and it takes it on one elevation at a time.

Thermal stress in low winter sun
When low sun hits part of a pane while the rest sits in the shadow of a frame, a storm panel or a deep eave, the two areas expand at different rates. Enough difference and the glass splits with nothing having touched it.

Seals worked across the full range
An insulated unit holds two panes and a gas fill together through humid ninety-degree Julys and sub-zero Januarys. That cycling, year after year, is why so many units fog well inside the life they were sold with.

Glass questions
Will a foggy window clear up on its own?
No. The seal has failed and moisture is permanently inside the cavity. It looks worse on humid days and better on dry ones, but the insulating value has gone.
Is defogging a real repair?
It can improve the view by drilling and venting the unit, but it does not restore the seal or the gas fill. We replace the unit, because that is the repair that holds.
Hail hit the house. How do I know what is damaged?
Look for pitting, small chips and cracks radiating from a strike point — then watch for fogging appearing over the following weeks, which means a seal took the hit even though the pane held.
My window cracked and nothing hit it.
Thermal stress, almost certainly. Low winter sun across a partly shaded pane is a common cause here and it is not something you did.
Can you replace one pane in a divided-lite sash?
Yes. That is exactly what divided-lite glazing is designed to allow, and the panes around it are not disturbed.
